OCANAT 10 ( obeticholic acid 10 mg )

Ocanat 10 is a generic brand of obeticholic acid used in India for the treatment of primary biliary cholangitis (PBC) in adults who are either unable to tolerate ursodeoxycholic acid (UDCA) or have not responded sufficiently to it. This therapy can be used in combination with UDCA or as a standalone treatment.

Obeticholic Acid Dosage

During the first three months of Obeticholic treatment, the prescribed dosage for non-cirrhotic or compensated Child-Pugh Class A patients should be 5 mg once daily. For Child-Pugh Class B or C patients or those who have experienced decompensation in the past, the recommended dosage is 5 mg once weekly.

If patients have not achieved an adequate reduction in ALP and/or total bilirubin after three months and are tolerating the medication well, the dosage may be titrated. Non-cirrhotic or compensated Child-Pugh Class A patients may receive 10 mg once daily, while Child-Pugh Class B or C patients or those with a prior decompensation event may begin with 5 mg twice weekly (at least three days apart) and titrate to 10 mg twice weekly based on response and tolerability.

The maximum dosage for Obeticholic is 10 mg once daily for non-cirrhotic or compensated Child-Pugh Class A patients and 10 mg twice weekly (at least three days apart) for Child-Pugh Class B or C patients or those with a prior decompensation event. It is crucial to recalculate the Child-Pugh classification before making any adjustments to the dosage. Decompensation events that require caution when adjusting the dosage include gastroesophageal variceal bleeding, new or worsening jaundice, and spontaneous bacterial peritonitis.
